Self-hosted PBX with a built-in CRM

A self-hosted phone platform for a multi-site clinic. Calls are routed where they should go, every conversation leaves a record, and operators work from a live dashboard instead of a paper log.

Who is it for?

This is for a clinic that runs several sites and needs them to answer as one. Patients call the clinic, not a location, so the routing has to reflect how the organisation actually works rather than which number happened to be dialled.

What it does

Self-hosted PBX

Runs on the clinic’s own infrastructure, built on Asterisk.

Multi-site routing

An IVR menu routes callers to the right site and department, so the conversation reaches the right place instead of depending on which number the patient happened to dial.

Emergency ring group

Urgent calls ring a dedicated group of handsets at once, so they never wait for a free operator.

Built-in CRM

Call history and callback requests live next to the phone system.

Real-time view of calls over WebSockets: the state of a line changes on screen the moment it changes in the exchange, with no reload.

Two-factor login

Operator access is protected with two-factor authentication.

Asterisk carries the telephony, Node.js and PostgreSQL sit behind the CRM, and the operator dashboard is React over WebSockets. The whole stack is packaged with Docker and runs on the clinic’s own infrastructure. That constraint shaped everything else: recordings and call history in a medical setting are sensitive, so self-hosting keeps both the data and the access rules inside infrastructure the clinic controls. The cost is that maintenance becomes the clinic’s responsibility, which is a trade worth making deliberately rather than by default.

Where it stands

In development, at the second of four stages, and not yet running in production. Built for one clinic rather than sold as a product, so there is no public installation to link to.
